• Physician Services – Hospice medical director and coordination with the patient’s attending physician 
• Skilled Nursing Care – Regular visits from hospice nurses for symptom and pain management 
• Medication Management – Medications related to the hospice diagnosis and comfort care 
• Medical Equipment & Supplies – Hospital beds, oxygen, wheelchairs, wound supplies, and other necessary equipment 
• Hospice Aide Services – Assistance with bathing, grooming, and personal care 
• Medical Social Services – Emotional support, counseling, and assistance with resources 
• Spiritual & Pastoral Support – Chaplain services for spiritual guidance and comfort 
• Volunteer Support – Companionship and assistance for patients and families 
• Bereavement & Grief Counseling – Support for families for up to 13 months after a patient’s passing
